講的是:echo沒有pericardial effusion(PEF),也不能排除Type A aortic dissection。

Fig.1 到院ECG
Fig.1 到院ECG(已裁掉抬頭個資)

心電圖顯示是sinus arrhythmia、1st AVB、LVH by voltage。

床邊可以用ADD-RS(Aortic Dissection Detection Risk Score)把這種感覺變成分數,看三類:

    • Marfan
AR murmur

❤️Case

Video 2|Parasternal short axis view
Video 3|Parasternal long axis view,

Video 5|
Video 6|

CT:Type A aortic dissection合併PEF(另有左側renal infarction)。

Video 8|CTA,
Video 9|CTA,
Video 10|CTA,

Table 1|
14分ketorolac 30 mg IV
15分76156/63morphine 5 mg IV
29分NTG(0.2 mg/mL)5 mL/hr起,4分鐘後調到6 mL/hr,目標收縮壓100–120;會診CVS

2024 ESC Guidelines for the management of peripheral arterial and aortic diseases建議:疑似急性主動脈症候群的病人,急診有條件的話就先做focused TTE,看PEF、wall motion、AR、主動脈直徑20。

2022 ACC/AHA

"In patients with AAS, initial management should include intravenous vasodilators if the BP is not well controlled after initiation of intravenous beta-blocker therapy."35

Table 2|
2022 ACC/AHA3960–80 bpm<120 mmHg,
2024 ESC40≤60 bpm<120 mmHg
Tintinalli 9th41≤60 bpm100–120 mmHg,

Fig.5 dP/dt

"There are no randomized studies that have evaluated different medical treatments in the treatment of AAS, although extensive clinical experience has established the current standard of anti-impulse therapy."44

Table 3|IRAD 2022,Type A早期死亡率

50 mcg/kg/min
150 mcg/kg/min
250–300 mcg/kg/min
  • Labetalolα+β,ESC
2 mg/min
  • Nicardipineβ-blocker

❤️Case

Fig.7 術後SICU ECG(已裁掉抬頭個資)

用短效esmolol(手邊沒有,才改labetalol或metoprolol)、小量滴定,NTG放後面甚至不用;一低血壓或已經填塞,就不降壓、不降心跳,改輸液加升壓劑、直接送開刀。
